PAYMENT POLICY

We will provide you with a written treatment plan and financial estimate prior to the start of treatment.  Our fees are a reflection of the quality of care we provide and are in line with the usual and customary rates for our area.  You are financially responsible for any and all charges for services rendered at Brecksville Dentistry for yourself and your dependents.  Any charges for services provided to a minor patient are the responsibility of the consenting parent or guardian who accompanies the child to our office for treatment.  We require your payment in-full at the time of service; and payment will be collected upon check-in.  For those patients with dental insurance, this includes but is not limited to any estimates for co-payment, co-insurance, and deductible that your insurance plan requires.

INSURANCE POLICY

As a courtesy to our patients with dental insurance, we will submit your claim and provide any necessary information to assist you in receiving your dental benefits.  It is your responsibility to know and understand your insurance benefits and to provide our office with accurate and current insurance information at least 3 business days prior to your appointment.  If your insurance has changed or terminated at the time of service, you are financially responsible for any and all charges for services rendered.  It is your responsibility to know if your insurance has any co-payment, co-insurance, deductible, prior authorization requirement, usual and customary limit, or any other type of benefit limitation; including but not limited to out-of-network coverage limitations. Please note that confirmation of benefits is not a guarantee of payment. If your insurance company has not paid the total claim within 60 days of your visit or pays less than we estimated, the total remaining balance will be your financial responsibility.
