Patient Responsibility: As the patient/guarantor it is your responsibility to know your insurance benefits and confirm with your insurance company that we are an in-network provider on your specific plan.
Brecksville Family Dentistry is committed to providing the highest quality dental care and our fee schedule is based on the usual and customary rates for our area. As the patient, you are financially responsible for any and all charges for services rendered. Please note that we require payment in full, including co-pays and deductibles, at the time of service and treatment will not be rendered until payment has been received. Payment for treatment provided to a minor patient is the responsibility of the parent or guardian. If a minor patient is not accompanied by an adult, payment must be sent with the patient.
It is your responsibility to know and understand your insurance benefits and to provide our office with accurate and current insurance information. If your insurance has changed or terminated at the time of service, you are financially responsible for any and all charges for services rendered. It is your responsibility to know if your insurance has any co-payment, co-insurance, deductible, prior authorization requirement, usual and customary limit, or any other type of benefit limitation. Please note that confirmation of benefits is not a guarantee of payment. If your insurance company has not paid the total claim within 60 days of your visit, the balance will be billed to you directly. Please note that if we receive a partial payment from your insurance company you will be responsible for any remaining balance.
